Below are links to documents you can download in prepration for competition as a varsity student-athlete at UMass Dartmouth during the 2009-2010 season.  The letter contains all of the information you need to know about what is required before you can play varsity sports at UMass Dartmouth. You can download the two forms, fill them out and return them to the UMass Dartmouth Training Room.

  All student athletes must fill out and submit the Medical History Form, the Insurance Verification Form and the Shared responsibility for sport safety form.

Student-athletes and parents also need to be aware of UMass Dartmouth's back-up insurance policy. Read this document for an explanation of insurance issues.
